When do I have my dog put down?

I have a peek-a-poo 16 yrs old. Everytime I start thinking its time to put her down, she perks up and seems playful and happy. However, she drinks water constantly, sometimes for 5 mins without stopping. Her hair is falling out, her paw pads are dry and peeling off, she has big round sores on both hind legs and hind leg weekness, and she shakes all of the time like she is cold. What do I do? Does anyone know what is wrong with her? Please help.
