Lab has difficulty opening mouth

My lab is 2 years old, and ever since he was a pup, he has had this problem with opening his mouth. He was treated with prednisone and antibiotic and the problem seemed to be resolved, but every couple of months the problem returns. Also the top of his head and face are sunk in, like there is no muscle tone. The vet is stump on what the problem is. He also has a thyroid problem which he takes synthroid for. Any suggestions.
